How to Fill UDID Card Online Registration Form

  • To begin with, go to the UDID Official Website
  • On the homepage, click on the "Apply for UDID" link present in the main menu or directly click https://swavlambancard.gov.in/Applyforudid
  • Read the steps to get your UDID Card and Disability Certificate, tick the declaration that you have read and understood the process and click "Submit" button.
  • Next the UDID Card Apply Online page will open.
  • Select any of the 6 options and click on the "Submit" button. For instance, we have selected 1st option.
  • Next the Aadhaar E-KYC page will open. 
  • Here you can select an option to either apply with Aadhaar number or apply with Aadhaar enrollment number. 
  • If you are applying with Aadhaar number, then enter it first and click "Generate OTP" button. 
  • Enter OTP received on your mobile number to validate it.  
  • Accordingly, UDID Card online registration form 2026 will appear as shown below:
  • Fill online form with personal details, disability details, upload photo/signature/thumb impression, proof of identity and address. Also fill in other asked details accurately.
  • Finally, click at "Submit" button to submit UDID Card online application form.
Note: Applicant should attend in-person medical board assessment for disability verification before final card issuance.

On the homepage, click on the "Track Your Application" link present in the main menu or directly click https://swavlambancard.gov.in/track-your-application
Upon clicking the link, the page to track your UDID Card Application will open.
  • Here you can track your application using either UDID number or mobile number or enrollment number or Aadhaar number.
  • For instance, we have selected Aadhaar number option to track UDID card status. Enter your aadhar number and click "Submit" button.
  • Subsequently, the page showing UDID Card status by aadhar number will get displayed on your screen.
  • Similar process is to be followed if you want to track UDID card status using other options.

Download Application / Receipt Online

  • Firstly visit the Unique Disability ID official website at https://swavlambancard.gov.in/
  • At the homepage, click at "PwD Login" link or directly click https://swavlambancard.gov.in/login
  • On clicking the link, PwD Login page will open.
  • Enter enrolment number/UDID number, date of birth, captcha and click "Login" button to open applicant's dashboard. 
  • Through this dashboard, one can update personal profile, download application or download receipt.
  • Download Application: If we click at "Download Application" option, then UDID Card PwD application will get downloaded on your laptop/PC/smartphone.
  • Download Receipt: If we click at "Download Receipt" option, then UDID Card Acknowledgement Receipt (Resident Copy) will get downloaded on your laptop/PC/smartphone.
